NHL: folliculair lymfoom graad 1 en 2

Stadium I-II

• Involved field radiotherapie.
• Eventueel expectatief (wait and see) of bij behandelindicatie chemoimmunotherapie zie bij stadium III/IV.

Stadium III en IV

• Expectatief beleid tot behandelindicatie ontstaat.
  Behandelindicatie bij beenmerg verdringing met cytopenie en/of aanwezigheid van een van de volgende criteria (GELF criteria):
  • Nodale of extranodale tumor met diameter > 7 cm
  • B symptomen
  • Verhoogd serum LDH
  • Aantasting ≥ 3 nodale lokalisaties (elk met een diameter > 3 cm)
  • Symptomatische splenomegalie
  • Compressie syndroom (ureter, galweg, trachea etc)
  • Pleurale, peritoneale effusie
• Jonger dan 65 jaar: In afwachting nieuwe trial: 8 x R-CVP¹
• Indien bezwaar tegen R-CVP:
  • Milde chemo-immunotherapie zoals chloorambucil (elke maand gedurende 14 dagen 1 x daags 10 mg totaal per os, gevolgd door 2 stopweken, tot maximale response, minimaal 6 maanden. Een ander schema is chloorambucil volgens St Barths schema: 6 wk lang 0.1-0.15 mg/kg dd, gevolgd door 6 mnd lang zelfde dosis gedurende 14 dagen gevolgd door 14 stopdagen). Chloorambucil combineren met Rituximab elke 14 tot 28 dagen totaal 8 x, of
  • Radiotherapie: lokaal 2 x 2 Gy of low dose TBI (total body irradiation).

NHL folliculair lymfoom, recidief

In afwachting van nieuwe trial:

• 6 R-CHOP₂₁, gevolgd door Rituximab onderhoud therapie, elke 3 maanden 375 mg/m² gedurende 2 jaar (gebaseerd op uitkomsten Intergroup EORTC 20981/HOVON 39 trial¹)
• Alternatief Fludarabine + Cyclofosfamide + rituximab (R-FC)
• Alternatief radio-immunotherapie Ytrium-90 Ibritumumab Tiruxetan (Y-90 Zevalin)
• Alternatief rituximab+bendamustine
• 2e of 3e recidief, overweeg ASCT of allotx
