Transfusies en bloedgroepwisselingen

Van alle patiënten die allogene hematopoietische stamceltransplantatie ondergaan wordt door BIBO-laboratorium vooraf uitgebreid onderzoek verricht van transfusie- antigenen (zie Rode bloedcel en HLA-fenotypering).

Na een ABO incompatibele stamceltransplantatie wordt volgens onderstaand schema 10 ml EDTA bloed ingestuurd tot definitieve bloedgroepverandering en transfusieonafhankelijkheid (in principe wordt dit via standaard lab pakket op afdeling en poli geregeld).

Tot 30 dagen:

1x per week ABO/D bepaling
1x per week anti-A/B titer
1x per week DAGT IgG, IgA, IgM, C3c, C3d, ctrl

Na 30 dagen:

1x per 2 weken ABO/D bepaling
1x per 2 weken anti-A/B titer
1x per 2 weken DAGT IgG, IgA, IgM, C3c, C3d, ctrl

Transfusie na allogene HSCT tot volledige bloedgroepverandering en transfusie-onafhankelijkheid

Patiënt krijgt altijd bestraald bloed (levenslang)
Patiënt krijgt altijd K negatief en rhesusfenotype compatibel met de donor
Onderzoek gedaan door BIBO:
- ABO/D patiënt
- Antistofscreening + Kpa
- DAGT IgG, IgA, IgM, C3c, C3d, ctrl
- Kruisproef

Hieronder volgt het schema voor de bloedtransfusie aanbevelingen zoals die door BIBO wordt gehanteerd.
